General Instructions

Please read carefully before proceeding with your registration:

1. Complete Online Registration
   Fill in the online registration form and submit it.
2. Print Medical and Transport Forms
   Print the Medical and Transport forms(If Opted).
   Fill them accurately with all required details.
3. Download the Required Forms

4. Prepare All Documents for Submission
   After online registration, take a printout of your registration form.
   Submit all three documents (Medical Form, Transport Form (If Opted), and Registration Form) along with:
   • A self-attested copy of your Aadhaar Card
   • ₹500 Registration Fee (One-time payment, to be paid at Silver Oaks Sports Academy)
   • Transport Fee (Monthly, If Opted)
   • Course Fee(Monthly)
5. Pending Payment?
   If you have already registered but your payment is pending, click here to Continue Payment
